The NHS Estate Challenge

The NHS manages over 29 million square metres of estate, costing over £10 billion annually. Yet, much of this estate is underutilised, outdated, or inefficient. The Darzi Report (2024) highlights how decades of underinvestment have led to a 'capital starvation' crisis, with mounting maintenance backlogs, inadequate clinical spaces, and outdated infrastructure.

The NHS Estates Return Information Collection (2024) further underscores these concerns, showing a growing gap between estate investment and service demand. Since 2021/22, the NHS maintenance backlog has increased by £2.1 billion, while available funding has fallen by £707 million in real terms. The result? Leaking roofs, inefficient layouts, and outdated IT systems that hinder staff productivity and compromise patient care.

Yet, amidst these challenges lies an opportunity. Many high-quality NHS buildings exist but are not fully optimised. Better utilisation of these spaces could ease pressure on hospitals, enhance patient access to care, and unlock cost savings.

Addressing the Barriers to Change

Several obstacles make estate planning and development within the NHS complex:

  • Insufficient Funding: Capital budgets are tight, with organisations competing for limited investment.
  • Complex Funding Landscape: NHS leaders often struggle to navigate funding streams effectively.
  • Cumbersome Approval Processes: Estates projects face up to 19 approval stages, causing delays and inefficiencies.
  • Workforce & Operational Constraints: Estates teams must balance maintenance, optimisation, and net-zero carbon targets.

Rethinking NHS Space Management

Given financial constraints, relying on large-scale funding for new builds is no longer viable. Instead, NHS estates teams must focus on maximising existing assets.

Key strategies include:

  • Optimising Existing Space: Increasing capacity through better scheduling and utilisation.
  • Enhancing Flexibility: Creating adaptable workspaces that evolve with service demands.
  • Leveraging Technology: Using sensor technology and data analytics to understand real usage patterns and inform decisions.
  • Repurposing Underused Spaces: Redirecting resources to the most efficient and effective facilities.

Case Study: Community Health CIC

One of our Community Health CIC clients faced major estate challenges, with key services operating in outdated and unsuitable spaces. A proposed new-build project was deemed unaffordable, stalling service improvements.

Through a collaboration between Baxendale and Abintra, we deployed occupancy sensors across multiple sites to analyse real-time space utilisation over a three-month period. This data-driven approach enabled the provider to:

  • Identify underutilised spaces for repurposing.
  • Develop a sustainable estates strategy without costly new builds.
  • Improve workspace efficiency for staff and service users.

Sensor technology provides real-time insights into estate utilisation, offering NHS organisations a powerful tool for strategic planning. By shifting from assumptions to evidence-based decisions, estates teams can:

  • Avoid unnecessary capital investment in new buildings.
